Job summary

AbilityFirst in Long Beach, CA is seeking a Community Navigator to provide person-centered planning, intake coordination and community engagement in our licensed programs.

You will collaborate with regional center service coordinators and families to ensure services align with participant goals, maintain documentation in ProCare and support outreach activities in the community. The role requires LIC Director Qualified and a valid driver’s license.

Qualifications

• High School diploma or equivalent is required.
• Bachelor’s Degree in psychology, sociology, etc or 4+ years related experience preferred.
• LIC Director Qualified in community center settings under Title 17/22 regulations is required.
• 2+ years experience in developmental disability services or related field is required.
• Familiarity with regional center processes, reports, and compliance is preferred.
• Strong written and verbal communication skills are required.
• Driver’s License is required.
• Ability to work with people of diverse backgrounds is required.
• Evenings/weekends may be required.
• Successful completion of background (LiveScan) check and vaccinations may be required.

Responsibilities

• Attend required Person-Centered Thinking training.
• Conduct screenings, phone calls and intake meetings.
• Document PCPs and reports (40+ pages annually).
• Coordinate with Regional Center Service Coordinators and families.
• Meet with participants monthly to review goals and placement.
• Coordinate community outings and volunteer activities.
• Input and maintain participant data in ProCare.
