Job summary

NLACRC is seeking a Service Coordinator to provide information, advocacy, and service arrangements for consumers and families. You will participate in program plan development, coordinate services, and document according to regulations and NLACRC policies.

The role requires meeting with individuals and families in homes, communities, or offices, and may include bilingual Spanish requirements depending on position. A California driver’s license and reliable transportation are necessary.

Qualifications

• Bachelor’s degree in psychology, social work, sociology, or related human services field with at least two years of related experience.
• Master’s degree in a related subject may substitute for experience.
• Understanding of family systems and person-centered planning; strong communication and organizational skills.

Responsibilities

• Provide information, advocacy, and service arrangement for consumers and families.
• Participate in development and evaluation of program plans for individuals.
• Identify and coordinate services for individuals and families.
• Document and complete forms and reports per NLACRC policies.
• Serve as an advocate for individuals served by community agencies.
• Schedule and attend interdisciplinary meetings with individuals, families, providers, and advocates.
• Identify barriers to service delivery and apply culturally sensitive strategies.
