Ascentria Care Alliance seeks a Community Supports Navigator to support individuals with ID/DD/ASD living at home or independently. The role involves developing plans, coordinating services, and teaching daily living skills while ensuring documentation and advocacy duties are met.

Candidate must be fluent in Spanish; a degree in human services and relevant experience are required. This is a full-time, hybrid position based in Worcester, MA with a strong commitment to community integration.

Qualifications

• Bachelor’s degree required with related human services experience.
• HS diploma with 10+ years of direct care experience considered.

Responsibilities

• Develop ISP, Behavior Plans and related assessments within deadlines.
• Provide training and support for daily living and health maintenance.
• Manage case work to promote independence and goals in ISP.
• Collaborate with leadership to meet rep-payee responsibilities.
