Join a multi-disciplinary team, participate in individualized service planning, and offer ongoing guidance. A high school diploma with experience or a relevant bachelor's is required, plus a valid driver's license and on-call availability.

Qualifications

• High school diploma with 3-5 years of experience or Bachelor's degree in mental health field.
• Driver's license with 3 points or less and access to insured vehicle.
• This role may have on-call hours.

Responsabilités

• Coordinating services for assigned clients, including psychiatric and medical care, housing, entitlements, etc.
• Developing individualized service plans with clients based on their goals.
• Providing ongoing and supportive assistance to clients.
