Qualifications

• High school diploma or GED required with 1 year of related experience.
• Bachelor’s degree preferred for the role.
• Willingness to obtain additional certifications and trainings as needed.
• Driver’s license with 3 points or less and access to insured vehicle.
• Availability to work days, nights, weekends and holidays as needed.

Responsibilities

• Assess and monitor client needs and develop service plans.
• Connect clients to community services and resources and advocate as needed.
• Assist with program enrollment and orientation for clients.
• Coordinate medical appointments, transportation, housing and money management.
• Develop and assist with medication management plans.
• Obtain and maintain entitlements for clients.
• Maintain accurate and complete documentation.
• Coordinate transitions plans and program discharge.
