Job summary

Texana Center in Rosenberg, TX is seeking a Care Coordinator to provide direct services to youth and adults with chronic mental illness, both in the community and on-site. Hours are 8:00 AM–5:00 PM, Monday–Friday, with schedule flexibility for after-hours appointments.

Qualifications

• Bachelor’s or advanced degree from an accredited college or University.
• Texas driver’s license in good standing as defined by Texana Center’s procedure.

Responsibilities

• Provide direct services to youth and adult individuals diagnosed with chronic, serious mental illness in the community and on-site.
• Develop the recovery plan and complete the ANSA or CANS assessments for patients on the caseload.
• Document and assess to meet applicable standards and performance benchmarks; work within a multidisciplinary treatment team.
• Coordinate and link patients to community resources and service providers; participate in crisis intervention and skills-building.
