Job summary

Wellvance Community Mental Health Authority is seeking a Community Living Support - Sub to help individuals build skills and independence in their communities. You’ll implement Person-Centered Plans, teach daily living tasks, support inclusion, and keep accurate documentation.

Flexible hours including evenings, weekends, and holidays are required, with travel within our service area and a valid Michigan driver’s license.

• Strong communication and organizational skills
• Flexible schedules and willingness to travel within service area
• Valid Michigan driver’s license and car insurance

Responsibilities

• Support individuals based on their Person-Centered Plan
• Teach daily living skills like meal prep, budgeting, and personal care
• Assist with community inclusion and social activities
• Advocate for individuals and promote recovery principles
