AHRC Nassau in Syosset, NY is seeking Direct Support Professionals to support individuals in Community Living Services, promoting independence and quality of life through daily living activities, health, safety, and social engagement.

You will work as part of an interdisciplinary team, follow treatment plans, assist with meals, medications per policy, transport to appointments, and mentor new staff while maintaining certification and training requirements.

Qualifications

• High School Diploma or equivalent preferred.
• Must have a valid NYS driver's license and maintain driver status.
• Ability to communicate and understand written and verbal directives.

Responsabilités

• Provide ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in daily life and independence.
• Assist individuals with self-care, health, safety, household chores, meals, and socialization.
• Follow treatment plans and document progress; support independence and wellbeing.
• Transport individuals for medical appointments, shopping, and outings.
• Mentor new Direct Support Staff and participate in house meetings.
