Responsibilities

• Provides ongoing guidance and support to individuals to ensure active participation in daily routines and wellbeing.
• Supports individuals in self-care, health, safety, meals, and community integration.
• Assists with medication administration and adheres to AMAP policy and procedures.
• Documents treatment plans, medical notes, and progress to promote independence.
• Provides transportation to appointments, shopping, and activities.
• Mentors new staff and participates in meetings to discuss progress and goals.
