We seek a Direct Support Professional who will provide direct support in residential settings, helping 4-6 individuals with daily living, health maintenance, and transportation.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.
• Valid driver’s license in good standing; vehicle insurance if transporting.

Responsibilities

• Deliver support per individual service plans, including socialization and health maintenance.
• Assist with daily living activities: personal care, housekeeping, recreation, transportation, shopping.
• Accurately document progress and activity; maintain confidentiality.
• Maintain professional relationships with individuals, families, guardians, and case managers.
• Report abuse or incidents and follow Sevita policy.
