Job summary

Mentor Community Services, part of the Sevita family, provides direct support in residential settings for individuals with intellectual and developmental disabilities. The role is non-exempt, hourly, with shifts that may be full-time or part-time to support a program.

You will deliver person-centered services, document progress, and ensure safety and confidentiality while supporting daily activities and community integration.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
• Makes cost-effective and appropriate purchases within the budget of the individual served and according to their choice / preference.
• Maintains healthy and professional relationships with individuals, families, guardians and case managers; follows Sevita Customer Service Standards.
• Reports any instance of alleged abuse or neglect; reports medical, behavioral and other incidents in accordance with Sevita policy and external requirements.
• Maintains confidentiality, respects the rights of persons being served, and assists individuals in exercising their rights.
• Accompanies individuals to medical appointments; relays orders and information to and from medical providers as required.
• Monitors individual’s health; documents health concerns; communicates with nurse or supervisor as appropriate.

Education

High school diploma or equivalent preferred

Six months of experience in human services preferred
