Job summary

Citizen Care Inc. seeks a Community Living Coordinator to oversee service coordination for adults with intellectual disabilities in residential settings. You’ll implement ISPs, ensure CQ Leadership Quality Measures, and coordinate with a multidisciplinary team while upholding the agency’s mission. Strong communication, writing skills and a valid driver’s license are required.

Qualifications

• Associate’s degree or 60 college credits, preferably in Human Services, with four years experience providing services and supports to people with intellectual disabilities.
• Bachelor's degree, preferably in Human Services, with two years experience providing services and supports to people with intellectual disabilities.
• One year experience in residential programs; prior experience developing and implementing ISPs for people with intellectual disabilities.
• Excellent communication and writing skills.
• A valid PA, Ohio or West Virginia driver’s license.

Responsibilities

• Coordination of services and supports
• Performing on-call responsibilities as assigned
• Focus on The Council on Quality and Leadership’s Quality Measures
• Compliance with CCI Policies and Procedures
• Compliance with 6400 Regulations and the Office of Developmental Program’s medication administration guidelines
• Performance of all ISP responsibilities of the Program Specialist position in accordance with the 6400 Regulations
• Quality Management Planning
• Incorporating the Agency’s Vision, Mission and Values
• Communicate effectively at all levels
