Job summary

Grafton Integrated Health Network is seeking a Direct Support Professional for Adult Services. You will support clients with daily living activities, follow treatment plans, and document progress while maintaining a safe, respectful environment.

Join a team dedicated to compassionate care, clinical excellence, and ongoing employee training. The role requires afternoon, evening, and weekend availability and offers an hourly wage of $16.00–$20.00 based on experience.

Qualifications

• Must be 18 years of age or older.
• High school diploma or GED required.
• Valid driver’s license with a clean driving record.
• Moderate computer skills including logging in, web navigation, and basic MS Office tasks.

Responsibilities

• Provide direct care and implement plans for clients as prescribed by physicians and treatment teams.
• Ensure supervision, safety and crisis management for clients.
• Document client behavior and participation in programming in a timely manner.
• Follow and understand Treatment Plans, IEPs, and ISPs for assigned clients.
