Grafton Integrated Health Network is seeking a Direct Support Professional for Adult Services. The role involves hands-on support, supervision, safety, and crisis management for adult clients, aligned with treatment plans and collaborative care.

Compensation is hourly and based on experience. Availability includes afternoons, evenings, and weekends, with comprehensive employee benefits and opportunities for growth within a non-profit setting.

Qualifications

• 18 years of age.
• High school diploma or GED.
• Valid driver's license with an acceptable driving record.
• Moderate computer skills - basic MS Office/web navigation.

Responsibilities

• Ensures the health, safety and wellness of Clients.
• Supports client health and hygiene and uses infection control procedures.
• Reports serious incidents and participates in reviews with follow-up measures.
• Follows and administers the individual plan, including goals and treatment strategies.
• Collaborates with the Treatment Team to provide continuity of support and care.
• Promotes clients’ achievement of behavioral goals by implementing strategies from the plan.
• Instructs clients to achieve education/residential goals and outcomes.
• Collects and documents behavioral and instructional data; documents services delivered.
• Maintains high engagement and monitors clients throughout the workday.
• Provides a supportive environment, building trust and positive relationships.
• Manages behavioral crises safely and oversees site management tasks.
