Job summary

Enhabit Home Health & Hospice is seeking professionals to support post-acute care navigation and transition planning. The role focuses on improving patient outcomes and referral satisfaction through evidence-based guidelines and coordinated care across health systems and physicians.

The position emphasizes collaboration, compliance with Medicare and regulatory guidelines, and a patient-centered approach within a supportive, growth-oriented environment.

Qualifications

• Must be a graduate of an approved school of nursing, therapy or social work.
• Must be licensed in the state where they currently practice.
• Must have two years' demonstrated field experience.
• Must have demonstrated knowledge of federal, state, and local laws and regulatory guidelines governing Medicare home health.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Assists patients in navigating post-acute care with the goal of improving patient outcomes and referral source satisfaction.
• Develops transition plans using evidence-based guidelines, preventative guidelines, and protocols to promote quality and efficiency.
• Represents the area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities.
