Responsibilities

• Build and maintain relationships with referral sources (physicians, hospitals, nursing facilities, and assisted living communities).
• Promote and educate partners on home care services, communicating value and identifying client needs.
• Serve as primary liaison between referral sources, clients, and internal teams to coordinate care.
• Ensure a smooth start of services through effective onboarding and communication.
• Maintain ongoing communication with partners to support satisfaction and address concerns.
• Represent the company in the community through outreach and in-home visits with prospective clients.
• Monitor market feedback and referral activity, providing insights to improve growth and marketing strategies.
• Stay informed on company policies, procedures, and industry trends; participate in team meetings to support goals.

About the Role

We are seeking a driven and relationship-focused Community Liaison to join our team in Panama City, FL. The territory includes the counties of Bay, Gulf, Calhoun, Washington and Holmes. This role is ideal for someone who thrives in a fast-paced, people-centered environment and is passionate about building strong partnerships within the healthcare community.

Key Responsibilities

• Build and maintain strong relationships with referral sources, including physicians, hospitals, skilled
• nursing facilities, and assisted living communities
• Promote and educate partners on home care services, clearly communicating value and identifying
• client needs
• Serve as the primary liaison between referral sources, clients, and internal teams to coordinate care
• and ensure a smooth start of services
• Maintain ongoing communication with partners to support satisfaction, address concerns, and
• strengthen long‑term relationships
• Represent the company in the community through outreach efforts and in‑home visits with
• prospective clients
• Monitor market feedback and referral activity, providing insights and recommendations to improve

Qualifications

• Sales or business development experience preferred, but not required
