Job summary

Swan Hospice is seeking a Community Liaison to serve as the primary link to community leaders, healthcare providers, and other stakeholders, building partnerships that support patient-centered care.

You will educate patients, families, and professionals about our services, participate in events, and work with the care team to ensure smooth transitions and coordinated care. Strong relationship-building and organizational skills are essential.

Qualifications

  • Bachelor's degree in a related field (e.g., social work, public health, healthcare administration).
  • Prior experience in community outreach, patient engagement, or healthcare administration.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Familiarity with hospice care and the needs of terminally ill patients and their families.
