Job summary

Swan Hospice in Monmouth County, NJ is seeking a compassionate Hospice Community Liaison to enhance coordination of care for terminally ill adults and support their families.

You will develop and maintain relationships with community partners and healthcare providers, coordinate patient-centered care, provide education and support to patients, families, and caregivers, and participate in case conferences to ensure continuous care.
