Job summary

Alliance Health in Charlotte, NC is seeking a Jail Liaison Care Coordinator to provide person-centered care transitions and care coordination for detained individuals. You will work with jail health staff and community providers to assess needs and support successful release planning.

This full-time hybrid role requires travel to detention facilities up to three times per week and occasional reporting to Alliance Home Office for meetings. Strong collaboration and documentation are essential.

Qualifications

• Diplomacy and discretion in handling sensitive information.
• Problem solving, negotiation, and conflict resolution skills.
• Ability to organize multiple tasks and priorities effectively.
• Proficiency with Microsoft Office applications.

Responsibilities

• Meet with members in jail to assess needs and participate in release planning.
• Document consent, coordinate care, and obtain releases of information.
• Develop member-centered care transition plans with the care team.
• Coordinate with jail and community providers to ensure smooth transitions.
• Travel to detention facilities up to three times weekly as required.
