Job summary

Alliance Health in Charlotte, NC seeks a Diversion Outreach Specialist to support members transitioning to community living, assess needs, and connect to services while collaborating with care teams to reduce barriers. This role also helps with data collection and program development for TCL initiatives.

The position requires a Bachelor’s or Master’s in human services or an RN with relevant experience, along with strong knowledge of community resources and excellent communication skills.

Qualifications

• Bachelor’s degree in a Human Services field and 5 years of relevant experience.
• RN with 3 years of experience working with individuals with SMI.
• Master’s degree in human services with 3 years of relevant experience.

Responsibilities

• Assist Diversion Clinician with transition to community from institutional care.
• Screen referrals and submit RSVP in a timely manner.
• Educate members and guardians about rights, options, and payer requirements.
• Collaborate with planning team to develop an accurate, person-centered plan.
• Travel throughout Mecklenburg County to meet with members and providers.
• Document in databases and maintain case records.
• Monitor and respond to changing member needs and service gaps.
