Job summary

PROVAIL is hiring a Community Inclusion Coordinator to support clients in developing personalized community inclusion plans and independent living skills. You will deliver inclusive activities for small groups and help participants engage in social, educational, and recreational opportunities within the community.

The role requires local travel, a valid WA driver’s license, and the ability to pass a criminal background check.

Qualifications

• HS diploma or GED completed.
• Valid Washington driver’s license with clean record and access to a vehicle.
• Willingness to pass a criminal background check.
• Lived experience and experience working with individuals with disabilities preferred.

Responsibilities

• Assist clients in developing individualized community inclusion plans and document progress.
• Provide community inclusion services for individuals or small groups to promote socialization and growth.
• Help clients identify activities and build skills for independence and participation in the community.
• Build relationships with clients and community partners to support participation and inclusion.
• Travel between work sites and participate in off-site meetings as needed.
