Job summary

Orchid Health is seeking a Community Health Worker to join our Sandy Clinic in Oregon. This 32-hour/week role focuses on helping patients navigate services, engaging families in care plans, and connecting with community resources to improve health outcomes.

You will work with the Care Management Team and providers, document interactions in the EHR, and build trust with patients in a rural community, with a strong emphasis on social determinants of health and patient advocacy.

Qualifications

• Minimum of high school diploma or GED; higher education preferred.
• One year of experience working with underserved populations in social services or community settings.
• Basic computer skills; familiarity with EHR is a plus.

Responsibilities

• Establish trusting relationships with patients and families and provide a welcoming, safe environment.
• Provide outreach to engage patients in care plans and connect them with community resources (SDOH).
• Support care management team in delivering whole-person care and coordinated services.
• Be a local liaison and patient advocate within the community.
