Job summary

Oak Street Health is hiring a Community Health Worker to bridge patients with community resources and medical systems. You will conduct proactive outreach, promote health literacy, and expand access to services to improve overall wellness.

The role requires flexible communication, problem-solving, and knowledge of the local community. 40 hours weekly in a full-time schedule with opportunities to travel across the community.

Qualifications

• Minimum 1 year of experience in healthcare, community-based, case management or social service.
• Strong oral and written communication skills are required.
• Ability to manage multiple priorities and stay positive.
• Access to reliable transportation and ability to travel throughout the community.
• US work authorization is required.

Responsibilities

• Establish and maintain relationships with patients and community partners to coordinate needs.
• Manage referrals defined by the care team and collaborate with the Medical Social Worker.
• Facilitate communication among patients’ families, providers, and community organizations.
• Build an inventory of local organizations that may benefit patients.
• Connect patients to state and local community resources for housing, transportation, food, and daily living needs.
• Assist patients with applications for benefits and resources.
• Promote goal setting and track progress with the care team.
• Meet patients in center, home, or community settings as appropriate.
