Job summary

Oak Street Health is seeking a Community Health Worker to bridge patients, community, and medical systems, removing barriers to wellness through proactive outreach. You’ll advocate for patients, coordinate care with medical social workers, and connect patients to state and local resources in a flexible, field-heavy role.

You will meet patients in Oak Street Health centers, homes, or community settings, travel within the community, and document interactions in the EMR while respecting HIPAA.

Qualifications

• Minimum 1 year of experience in healthcare or social services.
• Strong oral and written communication skills.
• Ability to manage multiple priorities while maintaining a positive attitude.
• Dedication to serving the community and building relationships.
• Proficient computer skills (Windows, GSuite, MS).
• Access to reliable transportation and ability to travel within the community.
• US work authorization.

Responsibilities

• Establish and maintain relationships with patients, community organizations, and team members to coordinate needs.
• Manage patient referrals defined by the care team and collaborate on action plans.
• Facilitate communication among patients, families, providers, and community groups.
• Build relationships with local organizations to benefit patients.
• Connect patients to housing, transportation, food, and other resources.
• Assist patients with benefit and resource applications.
• Promote goal setting to improve quality of life and self-efficacy.
• Meet patients at Oak Street Health centers, homes, or community locations.
• Spend roughly half time outside centers; reliable transportation required.
• Engage high-risk individuals and accompany to appointments as needed.
• Complete referrals, deliver health education, and support care team decisions.
• Document interactions in the EMR and maintain confidentiality.
