Job summary

Orchid Health is seeking a Community Health Worker in Sandy, Oregon, to support patients with social determinants of health and connect them to services. The 32-hour schedule is spread over four days, with Tuesday and Thursday as required days, at the Sandy High School clinic.

You will engage patients, coordinate with the care team, and help navigate resources to improve community health outcomes in a high-need area.

Qualifications

• High school diploma or GED is required.
• Understand rural communities served; preference via lived experience.
• One year experience working with underserved populations in social services or community settings.
• Basic computer skills; prefer experience with EHR.
• Ability to initiate and maintain positive relationships with patients and staff.
• Good communication skills and active listening.
• Ability to provide emotional support and motivation to patients.

Responsibilities

• Establish trusting relationships with patients and families; create a welcoming, safe environment.
• Provide initial outreach and engage patients in their care plan goals related to SDOH.
• Follow up on identified SDOH needs with patients and families; support care plans.
• Participate in an integrated care team for chronic care management and behavioral health needs.
• Coordinate services and develop care plans with the care team.
• Conduct outreach to new patients and those losing coverage to help establish care.
• Build relationships with community organizations and document contacts in EHR.
