Southeastern Idaho Public Health (SIPH) is seeking a Community Health Worker for the Nurse Home Visitation program. You will build relationships with mothers and provide practical resources to support their health throughout the first year postpartum.

The role offers a typical 40-hour schedule with a Monday–Friday clinic window (8 am–6 pm) and possible hybrid work after training. Compensation is hourly at $21.47, with benefits including paid leave and holidays.

Qualifications

• Six months experience in public health or a healthcare setting OR relevant coursework.

Responsabilités

• Build relationships with mothers and provide practical resources to support health in the first year postpartum.
• Collaborate with the clinical team to achieve program goals.
• Maintain client records and gather data, providing health and community program information to clients.
• Interview patients to obtain and document personal data including expected delivery date.
• Explain the program and tailor services to patient needs.
