Southeastern Idaho Public Health (SIPH) seeks a Community Health Worker for the Nurse Home Visitation program. You will build relationships with mothers and provide practical resources to support health postpartum.

This role emphasizes serving families across multiple locations in southeastern Idaho. The position offers a standard Monday–Friday schedule with 40 hours, possible hybrid after training, and opportunities for Public Service Loan Forgiveness.

Qualifications

• Six months experience in public health or healthcare setting OR relevant coursework.

Responsabilités

• Build relationships with mothers and families to provide practical resources for health postpartum.
• Collaborate with the clinical team to achieve program goals.
• Establish and maintain client records, gather data, and provide program information.
• Interview patients to obtain detailed personal data including expected delivery date.
