Job summary

Southwestern Idaho Public Health is seeking a Community Health Worker to support mothers and families in the Nurse Home Visitation program. You will build trusted relationships, document personal data, and connect clients with health resources.

A flexible schedule with clinic hours from 8 am to 6 pm is offered, with potential for hybrid work after training. The role emphasizes outreach, program promotion, data collection, and collaboration with clinical teams to achieve program goals in Idaho

Qualifications

• This position requires some knowledge of public healthcare practices.

Responsibilities

• Build relationships with mothers and their families.
• Gather client data and provide health and community program resource information.
• Interview patients to obtain and document delivery date and other personal data.
• Explain the program and adapt services to patient needs.
• Connect with patients between visits virtually or by telephone.
• Promote the program through educational campaigns and outreach.
