Job summary

Cleveland Clinic Health System is seeking a Community Health Worker Workleader in Rocky River, OH. You will provide leadership and training to CHWs, oversee daily operations, and coach a team to deliver a high-quality patient experience within the Pathway Hub.

This role requires strong communication, cultural mediation, and the ability to manage multiple priorities in a fast-paced healthcare environment. The position may involve travel up to 25% to support multiple sites, including OB practices,

Qualifications

• High school diploma or GED required.
• CHW Certification through the Board of Nursing required.
• Valid Driver’s license.
• Four years of CHW or patient navigation experience.
• Strong time-management and multitasking abilities.
• Ability to work in stressful or emergency situations.
• Excellent oral and written communication across diverse backgrounds.
• Ability to address cultural and social factors in health access.

Responsibilities

• Provide leadership and training to assigned CHWs.
• Oversee daily operations and conduct quality reviews.
• Coach CHWs to deliver positive patient experiences.
• Support EPIC documentation and achieve defined metrics.
• Travel up to 25% to support multiple sites.
