Job summary

The Cleveland Clinic Health System seeks a Community Health Worker Supervisor to lead, mentor, and coach the CHW team. You will carry a limited patient caseload and coordinate care with clinicians and social services to connect patients with resources.

Ideal candidates will have a Bachelor’s in a related field and several years of CHW or supervisory experience, with strong communication skills and a focus on patient-centered care.

Qualifications

• Bachelor’s Degree in Social Work, Human Services, Healthcare, or related field.
• Four years of CHW experience may be considered in lieu of degree.
• English required; Spanish preferred for some communities.

Responsibilities

• Provide supervision, coaching, and direct support to the CHW team.
• Lead onboarding, orientation, and training of CHW staff.
• Maintain a limited caseload while assisting patients to navigate community resources and services.
• Collaborate with medical providers, social workers, and case managers to meet patient goals.
• Serve as a liaison with community partners and stakeholders.
