Responsibilities

• Act as subject matter expert on CHW program workflow and care model.
• Carry a personal patient caseload and conduct social determinants of health interviews.
• Assist patients in overcoming barriers to health stabilization and access services.
• Lead onboarding, orientation, and training of CHW staff.
• Coordinate with Program Director and cross-functional partners.
