Zusammenfassung

MaineHealth is seeking a Community Health Worker to join the Lend a Hand program in our care management team. You will work with patients for about 6 months to overcome barriers, navigate the healthcare system, and advance health goals in a patient-centered setting.

The role is 40 hours per week, full-time, with a hybrid schedule (up to 2 days remote after training). A driver’s license and reliable vehicle may be required, with strong community knowledge and basic computer skills encouraged.

Qualifikationen

• Education: High school diploma or GED preferred.
• License/Certifications: Valid driver’s license, clean driving record, and personal vehicle if driving is required.
• Experience: Two years in a social service or health care setting preferred.
• Must reside in the community served with good knowledge of local resources.
• Basic computer skills preferred.
