Job summary

Hackensack Meridian Health is hiring Community Health Workers to provide social support, navigation, and advocacy for high-risk, low-income patients. You will perform social needs screening, community-based visits, and care coordination to help patients meet health goals while addressing social determinants of health.

The role requires driving in the New Jersey area, working in a hybrid setting, and collaborating with care teams.

Qualifications

• Bachelor's degree in health related field or 3+ years health or hospitality field with a High School diploma, GED, or GED-equivalent programs.
• Experience accessing social service resources available for New Jersey.
• Proficient in MS Word/Google Suite and computer data entry.
• Excellent oral and written communication skills.
• Ability to follow written and oral instructions.
• Ability to receive and address feedback.
• Ability to exercise judgment in applying professional services.
• Comfortable with telephonic outreach, home visits and community outreach.

Responsibilities

• Initiate contact with eligible patients telephonically and/or in person.
• Conduct social needs screening and document in the Electronic Medical Record (Epic).
• Provide telephonic, in-home, or community-based visits to assist patients with social and health-related needs.
• Conduct weekly follow ups with patients via their preferred method of communication.
• Meet enrollment targets as set by the department.
• Advocate for patients by connecting them to community resources and educating them on navigating them.
• Collaborate with care teams and community-based organization stakeholders.
• Assist patients in filling out applications for community programs and organizing paperwork.
• Prepare reports and documents as needed or requested.
• Attend regular team and departmental meetings.
• Work in a hybrid model setting as required.
• Adhere to organizational competencies and standards.
