1910 Cooley Dickinson Hospital, Inc. seeks a Community Health Worker - HIV Care to support HIV care and prevention, addressing social determinants of health and health equity. The CHW partners with clients to manage medical care, housing, and social services while collaborating with clinicians and APP teams to meet client needs.
The role involves needs assessments, health planning, advocacy, and ongoing follow-up to ensure continuity of care and improved health outcomes in a community setting.
The Community Health Worker - HIV Care is a key member of A Positive Place's HIV care and prevention team integrating community health work into clinical care by addressing social determinants of health and health equity. CHWs partner with individuals living with HIV in managing medical care, social services coordination, disease education, trauma and recovery, positive prevention, sexual health promotion, housing, and substance use risk reduction. The Community Health Worker - HIV Care works collaboratively with clients and other APP Care team members (housing, insurance, recovery coaching, peer advocacy, health navigation), as well as Infectious Disease specialists, PCPs, and other community programs to meet all needs.
