Job summary

The Brigham and Women's Hospital, Inc. is looking for a community health outreach professional to promote and maintain good health within communities. You will educate, support, and guide community members to reach their health goals, including navigation of the healthcare system and referrals to services.

You will perform health coaching, collect data, and participate in program development and trainings. Hybrid work with on-site duties at 3297 Washington Street and surrounding community sites.

Qualifications

  • High School Diploma or Equivalent required or Associate's Degree; Social Work preferred or Bachelor’s Degree in Related Field preferred.
  • Experience in community outreach, health education, or related field preferred (0-1 year).

Responsibilities

  • Conduct community outreach and engage community members to understand their health needs
  • Provide individual and group health education and coaching to community members
  • Assist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services
  • Provide basic health screenings and assessments to identify health risks and provide appropriate interventions
  • Assist in the development of health promotion programs and activities to address community health needs
  • Collect data and maintain accurate records of community health outreach activities and interventions
  • Attend meetings and trainings related to community health promotion and education

Job Summary

Responsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. Works under the direction of a healthcare professional to provide preventive care and health education to members of the community. Does this position require Patient Care? Yes.

Essential Functions
