Job Summary

Responsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. Works under the direction of a healthcare professional to provide preventive care and health education to members of the community.

Essential Functions
  • Conduct community outreach and engage community members to understand their health needs
  • Provide individual and group health education and coaching to community members
  • Assist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services
  • Provide basic health screenings and assessments to identify health risks and provide appropriate interventions
  • Assist in the development of health promotion programs and activities to address community health needs
  • Collect data and maintain accurate records of community health outreach activities and interventions
  • Attend meetings and trainings related to community health promotion and education
Qualifications

High School Diploma required. Associate’s Degree or Bachelor’s degree preferred. Experience in community outreach, health education, or related field strongly preferred. Bilingual (English/Spanish) strongly preferred. Pediatrics experience strongly preferred.

  • Strong communication and interpersonal skills, with the ability to interact effectively with various populations.
  • Demonstrated ability to work effectively and provide advocacy for all populations and communities preferred.
  • Ability to work independently and as part of a team.
  • Basic computer skills, including Microsoft Office and database management.
  • Bilingual skills preferred.
Additional Job Details

Schedule and Work Model: Monday through Friday ET, 8am-4:30pm or 8:30am-5pm. Must be flexible to support on-site needs at medical practices in Chelsea and Everett (approximately 2-3x/week but may vary based on case load) and at patient homes or schools for meetings. 2x monthly team meetings at Assembly Row in Somerville, MA office required (1st and 3rd Wednesdays of each month). Remote work requires stable, secure, quiet, compliant work station.

Remote Type: Hybrid. Work Location: 399 Revolution Drive. Scheduled Weekly Hours: 40. Employee Type: Regular. Work Shift: Day. (United States of America).

Pay Range: $23.51 - $25.28/Hourly. Grade: 3.
