Position Overview

We are seeking a dedicated Community Health Worker (CHW) to serve patients in the [LOCATION] area. The CHW acts as a liaison between health and social services and the community, facilitating access to services and enhancing the quality and cultural competence of care delivery.

Responsibilities

• Perform visits at ALF and ILF facilities within the designated service area.
• Serve approximately 200 patients, collaborating with the care team and health coaches.
• Provide patient/customer services, document findings accurately, and communicate effectively.
• Coordinate with Senior Health Express (SHE) to deliver a variety of services.

Job Details

• Full-time, salary-based 12-hour shifts (6:00 AM - 6:00 PM) on a 2,2,3 schedule.
