We are seeking a dedicated Community Health Worker (CHW) to serve our patients in the [LOCATION] area. A CHW understands the community well, acting as a liaison between health and social services and the community to facilitate access and improve service quality and cultural competence.

Community Health Workers perform visits at ALF and ILF facilities in their designated service area. This is a full-time, salaried position with 12-hour shifts (6:00AM-6:00PM) on a 2,2,3 schedule.
