We are looking for a dedicated candidate to join our team as a Community Health Worker (CHW) to service our patients in the [LOCATION] area. A CHW has a good understanding of the community served, acting as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
Community Health Workers perform visits to ALF and ILF facilities in their designated service area. This is a full-time, salary-based 12-hour position (6:00 AM - 6:00 PM) (2,2,3 Schedule).
