We are seeking a dedicated candidate to join our team as a Community Health Worker (CHW) to serve patients in the [LOCATION] area. A CHW understands the community well, acting as a liaison between health and social services and the community to facilitate access and improve service quality and cultural competence.

Community Health Workers perform visits at ALF and ILF facilities within their designated service area. This is a full-time, salaried position with 12-hour shifts (6:00AM-6:00PM) on a 2,2,3 schedule.
