Job Description

We are looking for a dedicated candidate to join our team as a Community Health Worker (CHW) to serve our patients in the [LOCATION] area. A CHW has a good understanding of the community served, acting as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

Responsibilities include:

• Perform visits to ALF and ILF facilities within their designated service area.
• Serve approximately 200 patients.
• Collaborate with the care team and health coaches.
• Provide patient/customer services.
• Document findings accurately and appropriately.
• Communicate effectively with patients and team members.

This is a full-time, salaried position with a 12-hour shift schedule (6:00AM-6:00PM) on a 2,2,3 schedule.
