Job summary

One Community Health is seeking a bilingual Community Health Worker - Connected Care to support frail older adults and their families in The Dalles, Oregon. You will help access information, connect with care teams, and coordinate services to meet patient goals.

The role emphasizes collaboration with primary care and behavioral health, with focus on medication reconciliation, advance directives, home safety, and community resource linkage. Standard hours are 8am–5pm, Monday–Friday.

Community Health Worker- Connected Care (Bilingual)

The Connected Care Community Health Worker plays an important role in helping older adults and their families access the information, resources, and coordinated care they need to achieve their individual care goals. Working closely with patients, their families, primary care providers, and behavioral health providers, this position helps ensure that what matters most to each patient is understood and communicated across the care team.

In this role, the Connected Care Community Health Worker supports patients with important aspects of their health and well‑being, including completing Advance Directives, reconciling medications, identifying and addressing fall hazards in the home, and connecting patients and families with community resources and services.

This position is part of an innovative pilot program designed to strengthen care and support for frail older adults. The Connected Care Community Health Worker will partner closely with primary care providers and the Connected Care team to implement, test, and refine pilot protocols while supporting an integrated, multidisciplinary approach to patient care. This position is full‑time, Monday‑Friday, from 8am‑5pm and located at our clinic in The Dalles, Oregon.

Minimum Qualifications:

• High School Diploma, GED, or equivalent.
• Bilingual in English and Spanish.
• Two years of experience in a health-related or social service-related field, or an equivalent combination of education and work experience.
• Current BLS/CPR Certification in accordance with OCH policy.
• Successful completion of credentialing and privileging in accordance with OCH policy.
• Strong written and verbal communication skills.
• Able to give presentations and provide instruction to patients and groups.
• Familiar with medical terminology and the healthcare system, including the Oregon Health Plan and Medicare.
• Understanding of the Community Health Worker scope of practice and willingness to seek appropriate assistance regarding medical recommendations.
• Knowledge of community resources within OCH service areas, especially resources available to older adults.
• Ability to work independently, manage multiple priorities and projects, and maintain a high degree of professionalism and confidentiality.
• Ability to work effectively as part of a multidisciplinary team in a diverse, multicultural environment.
• Ability to use initiative and sound judgment to resolve problems and challenges.
• Ability to remain calm and effective in high‑pressure and emergency situations.
• Strong organizational skills and the ability to manage multiple and competing timelines.
• High‑level computer skills, including Microsoft Office applications, Word, Excel, Outlook, Epic/OCHIN, and other systems.
• Knowledge of and compliance with HIPAA regulations.
• Knowledge of customer service and workplace safety.
• Ability and willingness to work in a dynamic and changing community healthcare environment.
• Valid driver’s license, proof of auto insurance, and clean driving record.
• Able and willing to travel to other OCH sites, community activities, trainings, home visits, and other designated locations, as needed.

Preferred:

• Advanced education, such as graduation from an accredited Medical Assisting program.
• 90‑hour Community Health Worker certification.
• Certified Community Health Worker.
• Experience working with low‑income, older adult, and diverse populations and their families.
• Experience or training in basic clinical procedures.
• Understanding of systems‑based work and HRSA requirements and expectations.

Compensation:

• Hourly/Non‑Exempt: $20.00–$25.00 per hour, depending on experience and certification(s).
• Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
• Mileage Reimbursement: In accordance with OCH policy.
