Job summary

One Community Health is seeking a bilingual Community Health Worker - Connected Care to support frail older adults and their families in The Dalles, Oregon. You will help access information, connect with care teams, and coordinate services to meet patient goals.

The role emphasizes collaboration with primary care and behavioral health, with focus on medication reconciliation, advance directives, home safety, and community resource linkage. Standard hours are 8am–5pm, Monday–Friday.

Qualifications

• Bilingual in English and Spanish is required.
• Strong written and verbal communication skills.
• Able to work independently and as part of a multidisciplinary team.
• Willingness to travel to other OCH sites and community activities.
• Must have valid driver’s license and clean driving record.

Responsibilities

• Help older adults and families access information, resources and coordinated care.
• Coordinate with primary care and behavioral health providers to align patient goals.
• Assist with Advance Directives, medication reconciliation and fall prevention planning.
• Connect patients and families with community resources and services.
