General Position Summary
This position is responsible to work collaboratively to assist individuals and families reach optimum health and wellness. Community Health Workers (CHW) provide care of coordination that includes assessment, planning, implementation, monitoring, advocacy and linkage to available services and resources. This position requires Oregon Health Authority (OHA) approved Community Health Worker (CHW) certification within 12 months of hire and on-going recertification every three years.

- A bachelor's degree in behavioral science, social science, or a closely related field; or
- An associate’s degree in a behavioral science, social science, or a closely related field and two years of human services related experience; or
- Four years of human services related experience.
- Satisfactory completion of OHA approved Community Health Worker training within 6 months of hire.
- Obtain Oregon Health Authority (OHA) Community Health Worker (CHW) certification and OHA Oregon Health Plan Assister Certification within 12 months of hire and on-going recertification every three years AND
- Reliable transportation: valid Oregon driver’s license with a driving record that permits coverage under the agency’s corporate auto liability policy; and personal automobile liability insurance.
Preferred Qualifications
- Experience conducting home visits, completing comprehensive assessments of need, and implementing service plans.
- Experience working in a team setting.
- Experience working with an electronic medical record or case management platform.
- Professional and/or volunteer experience that involves providing support, assistance, education, advocacy, or services that promote the social, physical, emotional, or economic well‑being of individuals, families, or communities experiencing high levels of risk.

Essential Position Functions and Key Work Processes
- Maintain OHA Community Health Worker certification and OHA Oregon Health Plan Assister Certification.
- Build and sustains strong relationships with community members and partners to identify priorities and mobilize action to improve health outcomes.
- Facilitate engagement activities, elevates community voice, and supports community‑led efforts that advance equity and access to services.
- Provide culturally responsive health education to support individuals, families, and communities in understanding health conditions, prevention strategies, and available services.
- Promote health and well‑being through outreach, prevention strategies, and collaboration that address social determinants of health.
- Provide care coordination that is safe, timely, effective, efficient, equitable, and person‑centered.
- Develop and implement service plans based on the information collected through assessments that identify the desired outcomes, goals, and support needs of the individual.
- Assist participants to access available services, including referral to related activities and resources that address identified needs in the service plan.
- Monitor services, including activities and contacts that are necessary to ensure that the service plan is effectively implemented and adequately addresses identified needs.
- Provide support and resources for individuals and families, maintain good working relationship with support networks, government resources, and community resources.
- Provide information, education, and technical assistance to facilitate informed decision making.
- Coordinate activities that support individuals and families and promote a sense of community.
- Advocate and raise awareness on behalf of individuals and families and the needs of the community.
- Organize and provide outreach in the community and with policymakers to support, develop and fund needed social work programs, policies, and services.
