COMMUNITY HEALTH WORKER/COMMUNITY PROGRAMS

This position is part of an Open Competitive Job Opportunity - Emergency recruitment with exam number Y8103K-EA. Applications will be accepted starting Wednesday, May 7, 2025, at 12:00 P.M. (PT) and will remain open until the needs of the service are met.

The Department of Health Services is seeking qualified candidates to fill emergency Community Health Worker vacancies related to the homelessness crisis, with the possibility of permanent employment following an initial assessment period.

About Housing For Health Program

Housing for Health provides housing and supportive services to homeless individuals with complex health needs. It is one of the largest homeless housing entities in LA County.

Job Responsibilities
  • Facilitate communication between the agency and the community, conveying policies and cultural attitudes.
  • Assist clients with obtaining benefits such as food stamps, housing, and healthcare services.
  • Advocate for clients to access departmental and community resources.
  • Help clients complete application forms for various benefits and services.
  • Provide emergency referrals and arrange shelter services.
  • Gather medical, mental health, and social histories, and assist with documentation.
  • Educate pregnant teenagers and their families about available health and social services.
  • Develop independent living skills through support groups and training.
  • Refer chronically ill children and their families to appropriate healthcare providers.
  • Screen referrals and coordinate counseling services.
  • Assist in research projects and supervise juveniles in detention facilities.
  • Perform basic clinical duties and coordinate tutoring or linkage services for youth.
Minimum Requirements

Option i: Six months of full-time experience working with the public or community groups in health or social services.

Option ii: Six months of experience as a Mental Health Advocate with LA County.

Applicants must meet one of these options to qualify. Part-time work will be prorated.

License and Physical Requirements

A valid California Class C Driver License or alternative transportation method is required. The physical class is Light, involving light lifting and considerable walking.
