This role involves working with STAR / CHIP members with Special Healthcare Needs, relaying pertinent information about their needs, advocating for optimal care, and ensuring access to necessary services.

Must reside in Texas and be able to travel to the local office/community about 10% of the time based on business needs. Telecommuting flexibility is available for residents within Texas.

Primary Responsibilities

1. Assess, plan, and implement individualized care strategies, aiming for the least restrictive, appropriate care level.
2. Identify and initiate referrals for social services, including financial, psychosocial, community, and state programs.
3. Manage care plans throughout the care continuum as the primary contact.
4. Communicate health-related information to stakeholders to ensure coordinated, quality care.
5. Advocate for patients and families to support their needs and choices within the healthcare team.

Required Qualifications

• High School Diploma/GED or higher
• 3+ years healthcare experience
• Proficiency in Microsoft Office (Word, Excel, Outlook)
• High-speed internet access and a dedicated, secure workspace at home
• Ability to travel within assigned regions up to 10%

Preferred Qualifications

• Certified Community Health Worker (CHW)
• Experience with Texas STAR and CHIP
• Case management and call center experience
• Experience with mental health members
• Bilingual in English and Spanish/Chinese/Mandarin

Salary ranges from $19.86 to $38.85 per hour, based on experience, location, and qualifications. Additional benefits and incentives are provided.
