**Position Overview:**

The In-Home Community Health Worker acts as a liaison between health/social services and the community, facilitating access and improving service delivery's quality and cultural competence. The role supports members in the in-home visit program by addressing health-related social needs, filling care gaps, facilitating primary/specialty care, and connecting to care coordination programs. This is a high-touch, field-based position serving members within Yakima County, primarily working remotely but requiring residence and travel within the region. The worker collaborates with internal teams and external partners to reduce barriers to care and enhance members' quality of life. Community Health Workers promote individual and community capacity through outreach, education, counseling, social support, and advocacy.

**Principal Duties:**

  • Engage plan participants identified through data analysis, including locating members in the home or community.
  • Conduct home visits to assess needs, connect members to care via telehealth or in-person, assist with transportation, educate on healthy behaviors, and inform about community resources.
  • Complete social driver of health assessments, identify social needs, and connect members to resources.
  • Coordinate appointments to address care gaps.
  • Consult with Community Health Centers and Care Teams to secure referrals for medical, behavioral health, and social services via outreach.
  • Provide updates on member progress and needs to relevant teams.
