**Position Overview:**
The In-Home Community Health Worker acts as a liaison between health/social services and the community, facilitating access and improving service delivery's quality and cultural competence. The role supports members in the in-home visit program by addressing health-related social needs, filling care gaps, facilitating primary/specialty care, and connecting to care coordination programs. This is a high-touch, field-based position serving members within Yakima County, primarily working remotely but requiring residence and travel within the region. The worker collaborates with internal teams and external partners to reduce barriers to care and enhance members' quality of life. Community Health Workers promote individual and community capacity through outreach, education, counseling, social support, and advocacy.
