Job description

Summary of Duties

As a Community Health Worker (CHW) based in the Puna area, you will provide vital assistance to clients in navigating the healthcare system while offering culturally appropriate education on chronic disease prevention, physical activity, and nutrition. Your work, rooted in the six pillars of CHW practice - respect, community base, capacity building, service to the underserved, social justice and health equity, and advocacy - aims to empower clients by enriching their health literacy and self-reliance. Collaborating closely with primary care teams, behavioral health specialists, and health educators, you will operate across clinical and community-based settings, including client homes.

**Due to the off-site/remote nature of this position, Community Health Workers must adhere to the following:

  1. Must have a dedicated, home office space clear of physical and noise distractions, with a power source, and that can accommodate a laptop and associated accessories.
  2. Candidates for this position must be based in the Puna area to execute client home visits within the district and surrounding areas as needed.
  3. Extensive travel within the assigned district is required.

Duties and Responsibilities

  • Conduct outreach in diverse settings such as homes, communities, and clinics, effectively communicating the program objectives and potential impacts on well-being.
  • Identify socio-economic determinants influencing clients' health and collaborate on comprehensive health and social management plans.
  • Maintain meticulous documentation of client encounters, ensuring accuracy and completeness; generate monthly encounter reports; maintain detailed electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client. Documents activities, service plans, and outcomes proficiently.
  • Coach clients in the management of chronic health conditions and inspire active engagement in self-care. Aid clients in understanding care plans and instructions.
  • Facilitate access to health-related services and advocate for clients' medical needs during visits.
  • Coordinate services between providers and clients, monitoring adherence to care plan objectives.
  • Expand knowledge of community resources, assisting clients in accessing essential services and support.
  • Cultivate positive relationships within the team and with stakeholders, striving to reduce barriers.
  • Travel extensively to client residences within assigned district, community venues, and agencies for outreach efforts.
  • Provide education on lifestyle modifications, emphasizing health promotion and disease prevention.
  • Collaborate with clients and families to address concerns and enhance service delivery satisfaction.
  • Builds partnerships to promote recovery, resiliency, and wellness among individuals and families.
  • Other duties as assigned.

Knowledge and Abilities Required

  • Strong interpersonal and communication skills to collaborate effectively with patients, clients, and agencies.
  • Organizational skills with the ability to multitask and maintain accuracy in documentation.
  • Knowledge of outreach programs related to healthcare delivery and clinical education.
  • Ability to communicate medical information to healthcare professionals and care coordinators over the telephone.
  • Creative and analytical thinking for planning, implementing, and evaluating client care plans.
  • Ability to work independently and in a team setting and able to multitask in a fast-paced environment.
  • Proficiency in office equipment and software applications, including electronic health records.
  • Must have experience collecting basic vitals (blood pressure, height, weight).
  • Ability to support CHWs in neighboring districts as needed.
  • Knowledge of the assigned district, or the ability to quickly become familiar with assigned district.
  • Must be ready and willing to travel to rural areas with uneven access roads and/or minimal signage and be able to navigate autonomously.

Minimum Qualifications

  • Education
    • High school diploma or equivalent
    • Community Health Worker certification (Must attach copy of certificate when applying)
  • Experience
    • Three years of general experience plus one year of experience directly related to the social work setting or relevant area, including duties such as but not limited to conducting client health assessments collecting vitals, and maintaining client records in the electronic health records system.
    • Associates degree in a relevant field may substitute experience requirement.
  • Other Certifications and Requirements
    • Community Health Worker certificate (or, currently enrolled in a program and expected to complete within 18 months of employment)
    • Current TB clearance (2-Step)
    • HIPAA Compliance Training
    • Bloodborne Pathogens Training
    • CPR/AED/First-Aid Training
    • Current State of Hawaii Driver License
    • Personal reliable vehicle with current insurance and registration (copies of these documents will be collected upon successful hire)

Preferred Qualifications

  • Familiar with HMONO's mission, programs, and services.
  • Possess a working knowledge of Native Hawaiian culture, traditions, history, and current events.
