Job summary

Moda Health in Oregon seeks a Case Manager to perform core case management activities, coordinate services, and monitor outcomes for members using motivational interviewing. This is a full-time work-from-home position with a salary range of $71,990.85 - $89,988.57 DOE; actual pay depends on qualifications and location.

Requires an accredited Nursing or Social Work degree and a current Oregon RN or LCSW license. CCMC certification or ability to obtain within 24 months is required.

Qualifications

• Graduate of an accredited school of Nursing or Social Work.
• Current unrestricted Oregon RN or LCSW license.
• CCMC certification required or ability to obtain within 24 months of hire.
• Recent healthcare experience in acute care and/or skilled nursing.
• Proficient with Microsoft Office; 35 wpm typing.
• Strong analytical and problem-solving skills.
• Excellent verbal and interpersonal communication; manage angry customers.
• Ability to work well under pressure.
• Professional image in person and on the phone; punctual.

Responsibilities

• Assess, plan, coordinate, monitor, and evaluate member needs and outcomes.
• Perform biopsychosocial assessments to identify needs and barriers.
• Develop individualized care plans with measurable goals.
• Coordinate services across behavioral health, medical, and social supports.
• Monitor progress and modify care plans as clinically appropriate.
• Utilize evidence-based practices to support care management.
• Engage members and families using motivational interviewing techniques.
• Collaborate with multidisciplinary teams to ensure coordinated care.
• Facilitate smooth transitions of care after hospital or ED visits.
• Identify candidates for case management via referrals and data.
• Serve as a behavioral health resource to stakeholders.
• Address claims/quality concerns with other departments as needed.
• Identify opportunities for process improvement and accreditation activities.
• Respond professionally to inquiries via written and verbal communication.
• Maintain timely, accurate documentation per policy.
• Plan, organize, and prioritize work to meet timelines.
• Other duties as assigned.
