Job summary

Mercy seeks a proven Supervisor for Community Health & Access to lead a team of Community Health Workers (CHWs) and directly support patients facing healthcare and social barriers. You will blend leadership, outreach, advocacy, and health navigation to improve access to care for underserved populations.

Qualifications

• Bachelor's degree in Community Health, Public Health, or a related field.
• 3+ years of CHW experience or equivalent experience.
• Certification in at least one chronic disease program (e.g., DPP).
• Basic Life Support (BLS) certification.
• Strong leadership and relationship-building skills.
• Ability to lead independently while guiding a team.

Responsibilities

• Lead and develop a Community Health Worker team.
• Coordinate daily operations, assignments, and priorities.
• Develop and deliver training programs and continuing education content.
• Support onboarding and professional development of new team members.
• Review documentation for quality, accuracy, and compliance.
• Partner with leaders to achieve community health goals.
• Provide direct services to uninsured and underinsured patients.
• Assist with Medicaid and financial assistance applications.
• Connect patients to primary care providers and resources.
• Address social determinants of health and connect patients to community resources.
• Manage patient caseloads and provide health coaching for chronic disease management.
• Represent Mercy in community coalitions and programs.
